About the Role

We’re seeking a Care Outreach Associate to serve as the compassionate front-line connection between Tembo Health and families navigating dementia as well as our partners.

This isn’t just a call center role—it’s an opportunity to be a trusted voice, a resource, and a guide for families who are often feeling lost in the healthcare system. You’ll educate, screen, and enroll individuals into our GUIDE dementia care program, ensuring they receive the right support at the right time.

You’ll also collaborate with Tembo’s interdisciplinary care team—Care Navigators, clinicians, and with our trusted senior living partners—to ensure each family’s experience is seamless and supportive from first contact through enrollment and beyond.

Key Responsibilities

  • Family & Patient Outreach: Conduct high-volume, empathetic outbound and inbound calls to individuals and families impacted by dementia who are interested in Tembo Health’s programs.

  • Education & Enrollment: Explain Tembo Health’s dementia care program, benefits, and eligibility—helping families understand the value of care coordination under Traditional Medicare.

  • Eligibility & Screening: Perform initial needs assessments to determine program fit in accordance with CMS GUIDE eligibility criteria (e.g., dementia diagnosis, Medicare Part A/B coverage, non-hospice, non-MA plan).

  • Scheduling & Coordination: Book assessments and clinical appointments with care team members, partner organizations, and senior living contacts.

  • Documentation: Maintain accurate, timely records of all interactions, assessments, and follow-ups in the CRM (HubSpot or EHR platform).

  • Collaboration: Partner with Care Navigators, clinicians, and senior living community liaisons to support smooth enrollment and care coordination.

  • Education Support: Help caregivers complete enrollment paperwork and understand available respite, support, and navigation resources.

  • Community Partnership Support: Conduct outreach and maintain relationships with senior living communities, home care agencies, and local dementia resource organizations.

Qualifications

Required:

  • Bachelor's degree or equivalent (MSW bonus)

  • Minimum 1 year of experience in healthcare, social services, senior living, or patient outreach

  • Excellent verbal communication and active listening skills

  • Demonstrated empathy and comfort engaging with individuals experiencing cognitive decline and their caregivers

  • Comfortable using virtual tools and CRM/EHR platforms for tracking and follow-up

  • Highly organized with strong attention to detail

  • Ability to work independently in a fast-paced startup environment

Preferred:

  • Experience working with seniors, caregivers, or dementia populations

  • Bilingual in English and Spanish

  • Familiarity with healthcare eligibility, Medicare/Medicaid programs, or CMS initiatives

  • Previous experience with telehealth, senior living outreach, electronic health records, or patient intake workflows
