The Role

Isaac Health is seeking a compassionate, proactive, and experienced Care Navigator Manager to lead our growing Care Navigation team. As part of our multidisciplinary memory care model, Care Navigators are the heart of patient connection—providing education, coordination, and support to people living with Alzheimer’s disease, dementia, and cognitive challenges, as well as their families.

In this leadership role, you’ll oversee the daily operations and performance of our Care Navigation team, guiding them to deliver exceptional patient and caregiver experiences, drive engagement with our services, and coordinate complex care with empathy and efficiency. You’ll collaborate closely with clinical leadership, patient-facing teams, and partner organizations to ensure patients feel supported every step of the way.

We’re looking for someone who thrives in a fast-paced telehealth environment, has a background in healthcare coordination or patient engagement, and knows how to lead with heart and hustle.

What You’ll Do

Team Leadership

  • Manage a rapidly growing team of Care Navigators and Care Navigator Admins (CNAs), including hiring, onboarding, coaching, and performance management
  • Facilitate regular team meetings, training sessions, and one-on-ones to foster connection and growth
  • Build a culture of accountability, compassion, and patient-centered excellence

Patient Coordination Oversight

  • Ensure timely and high-quality care coordination, appointment scheduling, and patient follow-ups
  • Support escalated patient concerns and guide Care Navigators through complex cases
  • Monitor SLAs and performance KPIs (e.g., monthly check-ins, documentation quality, time-to-appointment)

Process & Quality Improvement

  • Collaborate with Clinical Operations leadership to optimize workflows, documentation standards, and tech tools
  • Identify gaps or delays in patient support and implement systems to streamline efficiency and reduce care friction
  • Drive initiatives that improve patient and caregiver satisfaction, team efficiency, and care outcomes

Cross-Functional Collaboration

  • Liaise with medical providers, therapists, partner organizations, and tech teams to ensure seamless patient experience and operational alignment
  • Help execute clinical pilots, onboarding expansions, or operational enhancements as we scale

What You Bring

  • 5+ years of experience in care navigation, patient coordination, social work, or healthcare team management; required
  • Demonstrated success in people management and team leadership; required
  • Strong knowledge of dementia, Alzheimer’s, or complex/chronic care coordination; required
  • Bachelor’s or Master’s in a relevant field (e.g., Public Health, Social Work, Healthcare Administration, Nursing, Counseling); required
  • Proven ability to work independently and collaboratively in a remote, fast-paced environment
  • Proficiency with EHR systems, task management tools, and communication platforms
