About the Role
We're looking for a Software Development Engineer III to step into a critical architectural role at the heart of Innovaccer's provider-facing platform. This isn't a green-field feature team — it's ownership of live, production systems that power appointment scheduling, AI-assisted clinical documentation, and FHIR-based interoperability across multiple enterprise healthcare customers today.
You'll take over from a departing architect and become the go-to engineer for a set of services that directly affect how providers see patients, how clinical notes get created, and how data flows in and out of EHRs like Epic and Cerner. Alongside this, you'll help push the platform forward — building healthcare AI agents, contributing to RAG pipelines, and strengthening the FHIR/interoperability foundation that everything else is built on.
This role sits at the intersection of deep backend engineering and healthcare domain expertise. You'll need to be equally comfortable digging into a tricky production issue and designing a multi-tenant FHIR architecture that's built to scale over the long run.
A Day in the Life
Own core provider-facing services
- Act as the primary owner for the systems behind intelligent appointment scheduling, AI-assisted clinical documentation, and multi-tenant FHIR data exchange — the kind of services that, if they go down, a provider notices immediately.
- Triage and resolve issues in provider scheduling logic, working through complex provider hierarchies and edge cases.
- Support and evolve real-time data ingestion pipelines that power provider search and scheduling.
Lead FHIR & interoperability strategy
- Drive FHIR R4 and HL7v2 strategy in collaboration with InterOps teams and EHR vendors (Epic, Cerner, and others).
- Design and maintain multi-tenant FHIR architectures that handle user context propagation, audit compliance, and data isolation safely across customers.
- Troubleshoot EHR integration issues across multiple live customer environments.
Architect for scale
- Design scalable, secure, and reliable backend systems in Python (primary), with Go or Java where appropriate.
- Establish patterns for runtime configuration management across Python microservices.
- Build data routing and isolation mechanisms to keep multi-tenant systems safe and performant.
Build what's next
- Contribute to healthcare AI agents and RAG-based pipelines that bring AI directly into clinical and operational workflows.
- Partner with product managers, healthcare domain experts, and other engineers to translate real clinical and operational needs into shippable systems.
- Respond to customer escalations tied to scheduling availability or documentation delays, with urgency and ownership.
Raise the bar
- Mentor and guide other engineers; contribute to technical leadership and best practices across the data/backend engineering org.
- Own the end-to-end lifecycle of the tools you build — from prototype to production, including observability and long-term maintainability.
- Champion privacy-by-design and regulatory compliance (HIPAA) as a default, not an afterthought.
What You Need
Must-have
- 6–9 years of software engineering experience building distributed, backend systems (candidates with 5–6 years of strong experience will also be considered).
- Strong backend engineering experience — Python is preferred, but strong proficiency in Go or Java is also welcome if Python isn't your primary language.
- Strong database fundamentals (SQL and NoSQL) and RESTful API design experience.
- Hands-on experience with healthcare interoperability standards — FHIR R4, HL7v2 — and at least one major EHR API (Epic, Cerner, or similar) is strongly preferred. We know great backend engineers with this exact background are rare, so if you don't have direct healthcare experience but are a strong distributed-systems engineer willing to ramp up on the domain, we'd still love to talk.
- Experience designing for microservices, multi-tenant architecture, and service isolation.
- Strong system design skills (HLD/LLD) and sharp debugging instincts in production environments.
Good to have
- Exposure to AI agents, RAG pipelines, or LLM-based systems in production.
- Experience with provider scheduling logic or appointment optimization.
- Prior experience in healthcare SaaS or a health system IT department.
- Familiarity with modern data stack tools (Kafka, Spark, Airflow) and cloud platforms (AWS, Azure, GCP).
- Understanding of healthcare compliance practices — HIPAA, audit logging, PHI handling.
Education
- Bachelor's or Master's degree in Computer Science, Engineering, or a related field.
